The FA Criminal Records Bureau (CRB) Unit is now using a new application form. This form is purple and has been in use since 1st July 2010. All white CRB forms should be disposed of.

This is just a new application form. Club Welfare Officers need to continue to verify CRB forms of those volunteering/working with children who are new to football and renew individuals’ CRB Disclosures every three years.
